IMPORTANT INFORMATION
This ‘educational’ skin check will be provided free of charge to you (the participant) with all reasonable care.
It's important to remember due to the nature of skin cancer our team can't guarantee you don't have skin cancer at the time of assessment, or that you will not develop skin cancer in the future. Please let our practitioner know of any moles / spots you're concerned about. Also, it is recommended to undress to your underwear for your assessment, although you are entitled to a partial check if you prefer.
To ensure accurate care and monitoring, lesions of concern are photographed by your practitioner. These images and the information you provided is safely stored by DermEngine / MoleScope and will be accessible to you at any time through your profile. The information about your consultation may be accessed by authorised staff (and potentially authorised Government bodies) appropriate to your care. We will only use this information for the intended purposes of assessment, improving our services and/or your treatment. We may email & SMS you prior and post appointment, and potentially share de-identified lesions to our social media.

ACKNOWLEDGEMENT
I have read and acknowledge the above 'important information' and understand that I take full responsibility to see my local skin clinic / GP / dermatologist to follow up on any suspect lesions as recommended by the nurse or to seek a second opinion which we always encourage. I authorise the specialist to release or obtain medical information to or from the referring doctor, general practitioner, insurance company, emergency contact, solicitor or other health professionals in matters concerning my skin health. Also, your Specialist, Skin Check Champions and all our associated parties are not liable for any misdiagnosis or failure to follow up if requested. This is an educational exercise, we'll do the best we can and we always recommend you see your GP as soon as you notice any changes / abnormalities and to seek second opinions.

WHAT IS INVOLVED?
You will be offered a free, private 15-minute full body skin check provided at the ‘pop-up’ clinic run by Skin Check Champions. This service will be provided by a nurse using a dermatoscope (a magnifying and light source visual aid). The nurse dermatoscopist is trained and experienced in assessing the skin for abnormal skin lesions that are suspicious for skin cancer.
Prior to attending this appointment, you will be invited to create a free account with ‘MoleScope’ and answer a simple questionnaire regarding your skin and sun exposure history. From this information your dermatoscopist will be able to tell you what your risk profile is for skin cancer (melanoma and non-melanoma). Details required to create your MoleScope account will include: name, address, phone number, email and date of birth. Your account will only be accessible by yourself, Skin Check Champions, your dermatoscopist and your medical practitioner should you require further investigation of any abnormal skin lesions. All information is securely stored, and password protected on the MoleScope platform that Skin Check Champions and its partners uses to manage its clinic and service to clients. During your private 15-minute skin screening appointment, you will be required to remove your clothing (except for underwear). Your dermatoscopist will perform a full body skin check and will include all areas of the body including the scalp, the palms, and soles of the feet and between all fingers and toes. If you are concerned about a skin lesion located below your underwear, please inform the dermatoscopist and they can discuss this with you.

PARTICIPANT CONSENT FORM
Our dermatoscopist will use an instrument called a dermatoscope. The dermatoscope has illumination and magnification and allows the nurse to look for abnormal features found below the surface of the skin. The dermatoscope is utilised to assist in recognising malignancies at their earliest stage. There is no discomfort associated with the use of this instrument, it does not touch your body. Your dermatoscopist will provide you with educational information regarding prevention and detection of skin cancer. If they identify anything abnormal on your skin, they will take an image with the dermatoscope which goes directly into your MoleScope account. This will serve as a baseline of your individual skin lesions for monitoring change & will assist your doctor for further investigation if required.

ARE THERE ANY RISKS?
Aside from your time, we do not expect that there will be any risks associated with receiving a skin check. If a suspicious lesion for skin cancer is identified, the image will be photographed, and it will be discussed with you how best to follow-up with a doctor for further investigation, diagnosis and treatment if required.

WHAT HAPPENS IF SOMETHING ABNORMAL IS FOUND?
Any skin lesions that your dermatoscopist deems as being abnormal, will be run through the MetaOptima's artificial intelligence (AI) system. This may be used as a second opinion in helping to determine whether the lesion may be a skin cancer or not. This information will also be accessible to your GP or dermatologist should they require it. At the completion of the skin check, the nurse dermatoscopist will summarise your results. If you have any abnormal skin lesions, your results will be forwarded to a GP of your choice and will be given access to your MoleScope account ie. This will assist with continuity of care. The nurse dermatoscopist does not provide you with a diagnosis, they can only advise you that the skin lesion is abnormal and you should have it followed up with a GP or dermatologist. Your options regarding advice of who to see will be discussed with you at the time.

WHAT HAPPENS IF NOTHING ABNORMAL IS FOUND?
Depending on your skin type and other risk factors, regular skin checks are suggested by the Royal Australian College of General Practitioners. The frequency of these skin checks will depend on your overall risk and this should be discussed with your GP or other qualified health practitioner. It is also recommended that people are skin aware, undertake self-examinations on a regular basis and consult with a GP or dermatologist should they notice any changes or abnormalities.
